NYSDA offers its members a registry service for educational credits earned each year. The registry electronically records continuing education (CE) credit hours for courses completed and provides participants with an annual transcript of their continuing education activity.
Why is the NYSDA Registry Important?
Your NYSDA transcript is accepted by New York State as proof of completion of all courses listed. Malpractice carriers, hospitals and other employers also accept the NYSDA transcript as proof of continuing education activity.
The Registry documents courses submitted for credit up to one year after completion of courses. This service eliminates much of the paper work now mandated by law.
How Does the Registry Program Work?
Members are responsible for mailing their own course submissions to: NYSDA Registry, 20 Corporate Woods Blvd., Suite #602, Albany, NY 12211. Only your local dental society will automatically provide documentation to the Registry for courses they have sponsored. Other course sponsors should provide you with a certificate or letter of completion for you to submit to the Registry.
At the end of each year, members receive a transcript detailing the courses submitted and the CE credits earned. View transcript online--members only.
Each submission must include:
- Dentist’s name & ADA number
- Course title or code number
- Sponsor name
- Date of completion
- Number of course hours
- Authorized signature (if imprinter is not used) by course sponsor
