NYSDOH Issues MAX Series High Utilizer Report

The New York State Department of Health (NYSDOH) has issued a report of the most recently completed Medicaid Accelerated eXchange (MAX) Series.  Between January and July 2017, Action Teams across New York State participated in the MAX Series, which focused on improving care for High Utilizer patients, with a goal of reducing inpatient admissions by 10% over a six month period.  Over the course of the MAX Series, each of the 22 participating Action Teams developed a High Utilizer care pathway, and measured process improvements to achieve change within their local hospitals and communities.  Included in the report is an overview of the MAX Series methodology, key considerations for High Utilizer care pathway development, and measurable results achieved by the participating Action Teams.  To read the NYSDOH MAX Series High Utilizer report, use the link below.
